Reconnect is a youth-focused prevention and intervention program for individuals aged 13–18 who are at risk of homelessness or experiencing street involvement.
This is often alongside concurrent challenges such as mental health concerns, substance use, and family conflict.
Through outreach, individualized support, and resource navigation, the program helps youth access housing, education, and essential services, while promoting housing stability, reducing high-risk behaviours, and improving family relationships through parent-teen mediation, with crisis intervention available to address urgent needs and support overall well-being.
