The Outreach Mental Health Program supports youth aged 12–18 who face high-risk mental health challenges and barriers to accessing traditional services.
Serving youth who are often homeless or at risk of homelessness, use substances, and lack personal support networks, the program provides non-intrusive care tailored to their needs. Staff work collaboratively with youth to assess needs, create and monitor treatment plans, provide crisis intervention, and connect them with mainstream mental health resources, with referrals accepted from Child and Youth Mental Health.
